Free Physicals for Sports

Physicals 2011
If you have any questions you may call the CNS Health Office at 218-4103 or the NSJH Health Office at 218-3603.
Physicals are provided for students:
- Participating in interscholastic activities
- Needing working papers

Needing school physicals for grade 10 or new students
The green physical form may be picked up in any school Health Office, from the Directors' Office or downloaded below in FORMS. The Health History side of the form must be completed, signed, and dated by a parent/guardian for the student to have the physical at school.
If you have a special student who may need clearance for the Selective Classification contact the ATHLETIC DIRECTOR, Mr. Tatham at CNS HS. All appropriate forms must be completed by the parent and presented to the physician at the physical. Remember --this is for a few very select student-athletes.

Health Office Approval (submit 2 copies)
All of the forms require parent signature.
* Completed Physical Form & Health Office Approval Form should be submitted to the School Nurse
*Completed Code of Conduct forms are to be submitted to the Coach
